Palm Beach District Requirements for Back to School!
August 1, 2016Students entering kindergarten and seventh grade, and new students to Palm Beach County are required, by the school district, to provide proof of immunization and a physical exam. In addition, children entering or attending licensed facilities for child care and child care homes should have received the greatest number of appropriate immunizations as medically indicated in accordance with the recommended immunization schedule for children. Florida Certificate of Immunization, DH Form 680, is the only document that schools are allowed to accept as proof of immunization. In addition, the DH 3040 form (yellow form) is allowed as evidence of physical examination. Call your Pediatrician for more information and details.
- Diphtheria/Tetanus/Pertussis (DTaP) – 5 doses
- Polio (OPV or IPV) – 4 doses
- Measles, Mumps, Rubella – 2 doses
- Hepatitis B (Hep B) – 3 doses
Kindergarten to 8th Grade:
- Varicella – 2 doses or verification of disease history (Form DH 680)
9th – 12th Grade:
- Varicella – 1 dose or verification of disease history (Form DH 680)
7th – 12th Grades (entry or repeating, if not given previously):
- Tetanus/Diphtheria & Pertussis booster (Tdap)
Pre-K Programs:
- Diphtheria/Tetanus/Pertusis (DTaP)
- Polio (OPV or IPV)
- Measles, Mumps, Rubella (MMR)
- Haemophilus Influenza, type B (Hib)
- Hpatitis B (Hep B)
- Varicella (VZV)
